Formstack Forms is a browser-based form creation tool. Forms can be shared as standalone pages or embedded in existing web pages.

If the data you're collecting is protected under GDPR, you'll need to ensure that submission data is only shared with the people who need to see it. You'll also need a plan in place for deleting the data once you reach the deadline for retaining the information.
