Formstack

Formstack Forms is a browser-based form creation tool. Forms can be shared as standalone pages or embedded in existing web pages. 

Key features

  • Simple drag-and-drop form builder.
  • Create complex, multi-page, multi-threaded forms using logic to show and hide fields depending on selections in other fields.
  • Connect form submissions with Google Drive and Google Sheets to collect data and submitted files.
  • Automate submission messages for users and notification emails for form administrators.

Service commitments

If the data you're collecting is protected under GDPR, you'll need to ensure that submission data is only shared with the people who need to see it. You'll also need a plan in place for deleting the data once you reach the deadline for retaining the information.

Support

  • This service is managed by a third party.
  • They are responsible for monitoring, identifying and fixing faults. We provide advice and support for local issues during our opening hours.

Standards

  • Our service performance and standards have been produced in consultation with our customers, and we monitor the delivery, performance and availability of facilities and services.
