How to get involved

A service evaluation is being run across the Humber and North Yorkshire region. Your link worker/support worker will ask you if you would like to take part in the service evaluation as part of your ‘What matters to you’ conversation.

The link worker/support worker will talk you through what this service evaluation is about and also support you in filling out the questionnaires.

Before you take part in the service evaluation, you will fill out a consent form to give your permission to take part. All information you provide on the questionnaire is anonymous. In other words, you will not need to provide any personal information, such as your name, and any information that might identify you will be removed, so you cannot be recognised.
